We are now requiring all prescriptions to be sent directly to your pharmacy of your choice. Please nominate your pharmacy when making your request.
If you do not specify a pharmacy, we will send the prescription to the Unichem Kopata Pharmacy located next door to our practice.
If you are ordering on behalf of someone else, please also include your name and relationship to the patient for whom the prescription is being ordered.
All prescription requests for patients fourteen and over will incur a fee. Prescriptions that are ordered but not collected will still incur a full charge.
The issuing of prescriptions is at the doctor’s discretion, and they are required to follow the current legislation and recommended best practice.
If you have not seen your doctor within the last six months, you may be asked to attend a consultation with your GP prior to obtaining your prescription.
